Back Acne on the Rise: Dermatologists Address Causes and Care

While facial acne often gets the spotlight, dermatologists are seeing a growing number of patients struggling with large, persistent breakouts on their backs — a condition widely referred to as “bacne.”

“Back acne is typically more severe than facial acne because the skin on the back is thicker, and the pores are larger,” explains Dr. Sarah Lopez, a board-certified dermatologist. “This makes clogs deeper and inflammation harder to treat.”

Experts point to several triggers: excessive sweating, tight or non-breathable clothing, poor hygiene after workouts, and even hormonal imbalances. In many cases, heavy backpacks or sports gear also contribute by trapping heat and friction against the skin.

Big acne lesions on the back are often painful, slow to heal, and prone to scarring or dark spots. Doctors recommend daily cleansing with gentle, non-comedogenic products, regular exfoliation, and avoiding oily body lotions. Treatments such as benzoyl peroxide washes, salicylic acid sprays, or prescription oral medications may be necessary for stubborn cases.

“Back acne can take a real toll on confidence, especially during summer when people want to wear lighter clothing,” Dr. Lopez adds. “But with the right approach, it’s absolutely treatable.”
